Immunization Records
You need to plan on visiting UT Medical Group prior to the beginning of the school year so you can have your children examined, vaccinated, and cleared to begin school. We will provide you a copy of your child's immunization record. Please bring this record with you each time you return to the clinic and it will be updated.  A copy of this record may be obtained for $5.00. Call the Pediatric HIM department to request another card.

Birth and Death Certificates
Birth and death certificates are not routinely maintained in UTMG records. To obtain an official copy of these documents, contact the Health Department in the county of your residence.
